Attention New Clients:

  • The CCA New Patient Intake Form and a copy of your insurance card will be required three days prior to your appointment. You can find the CCA New Patient Intake Form on our “patient forms” tab.
  • New Clients For Medication Management: If you have been prescribed psychiatric medication in the past, records must be sent to our office prior to your initial appointment. If records are not received, changes to medication may not be made until records are received.
  • Patients needing ADHD testing must be referred by a physician/certified provider. Records must be received prior to scheduling an appointment.
  • Pediatric patients under the age of 16 must have a parent/guardian present at each appointment unless approved by their Clinical staff member.

Important COVID-19 Updates

Compass Clinical Associates will re-open for in-person appointments on April 15, 2021. Tele-practice will remain an option for clients so long as insurance will cover tele-practice sessions. It is the responsibility of our clients to know if their specific insurance coverage will pay for specific services. Claims denied by insurance are the responsibility of the client. We strongly recommend that patients contact their health insurance provider to know what insurance will cover. Patients should check on the continuation of tele-practice coverage. Any services not covered by insurance are the patient’s responsibility to pay.

We ask that everyone who comes inside the building wear a mask. We apologize for any inconvenience, but our number one priority is keeping everyone as safe as we possibly can. If you have any questions about this new protocol please feel free to call our office at (515) 412-5112.

If you are needing to send a picture of your insurance card, send medical forms to your provider, or any other requests, please go to the “contact” tab and fill out the Contact Form. You will be able to attach your document/picture if needed then hit submit. Your message and attachments will be submitted to our office and we will be in contact with you.

COVID-19 Precautionary Measures

Compass Clinical Associates re-opens for in-person appointments on April 15, 2021. Because your safety and comfort are our priority, we have taken extraordinary safety precautions related to COVID-19. These precautions include:

  • Our entire office is being professionally cleaned three (3) times a week.
  • Lobby areas are sanitized twice daily (morning and afternoon). We sanitize pens after each use. We sanitize door handles and all commonly touched areas.
  • Seating in our lobby area reflects social distancing: seating is spaced six feet apart.
  • Employees and clients are required to wear masks.
  • We have multiple hand sanitizing stations available.
  • We have installed Plexiglas dividers at the check-in windows to add an extra barrier for patients, clients, and staff.
  • We have signage throughout the building reminding patients, staff, and clients of mask wearing, social distancing and to not enter our building if you are not feeling well/experiencing COVID-19 symptoms.
  • Thermometers are available for use as needed.
