Attention New Clients:

  • The CCA New Patient Intake Form and a copy of your insurance card will be required three days prior to your appointment. You can find the CCA New Patient Intake Form on our “patient forms” tab.
  • New Clients For Medication Management: If you have been prescribed psychiatric medication in the past, records must be sent to our office prior to your initial appointment. If records are not received, changes to medication may not be made until records are received.
  • Patients needing ADHD testing must be referred by a physician/certified provider. Records must be received prior to scheduling an appointment.
  • Pediatric patients under the age of 16 must have a parent/guardian present at each appointment unless approved by their Clinical staff member.
