Call our office at 515-412-5112. Our administrative staff can confirm your insurance information and process credit card payments.
Check in with the front desk. Our administrative staff will make a photocopy of your insurance card and review your information. If you have completed forms prior to the appointment, please give them to our support staff at the front desk at this time.
