Step 1: Check Your Insurance

Call the 800 number on your insurance card to learn about your mental health benefits. Key questions to ask:

  • Is mental health treatment covered?
  • Do I need pre-authorization?
  • What is the co-pay?
  • Does my policy cover the provider I choose?

Step 4: Complete Patient Forms

Complete the required forms online or in-office (allow 15 minutes).

Arrive 10 minutes early with:

  • Completed forms (if done prior)
